Background

Population health management (PHM) strategies have the potential to improve thequality of CKD care, but significant barriers remain to leveraging existing healthinformation technology (health IT) and electronic health record (EHR) data for PHMin CKD. Additionally, efforts to overcome these barriers and implement PHMstrategies for CKD often are developed in isolation, with limited opportunities toshare best practices and lessons learned.

Meeting Objectives

To advance PHM approaches to improve kidney health, the NIDDK will host aconference to identify best practices for using the wealth of information withinexisting EHRs and health IT systems to deliver optimal care and manage CKDpatients in real time. Accordingly, this conference aims to:

  • Review best practices for kidney-related PHM efforts for research (e.g.,pragmatic trials) and clinical care (e.g., PHM implementation across health caresystems) from experienced centers across North America.
  • Review challenges and opportunities to facilitate additional broad-scalepopulation efforts for research and clinical care for kidney disease.
  • Develop a repository to aggregate shared strategies and resources (e.g.,algorithms, code) that will facilitate kidney disease PHM efforts for research and clinical care.
