October 22 and 23, 2015 - Population Health Management Meeting (PDF, 295 KB)

The HIT Working Group collaborated with the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) to convene informaticists, clinicians, and patients at the Using Health Information Technology to Identify and Manage CKD Populations meeting. The meeting aimed to identify challenges and strategies for managing CKD populations using HIT.

November 22, 2013 Working Group Meeting

The HIT Working Group discussed the Working Group's draft paper on the incorporation of CKD data into electronic health records, proposed clinical quality measures for CKD, and getting data to patients as a next step for the Working Group.

March 8, 2013 Working Group Meeting

The HIT Working Group discussed partnering with NKDEP's Pharmacy Working Group on an e-prescriptions effort, incorporating CKD data into EHRs, and priorities for the Working Group.

October 31, 2012 Working Group Meeting

The HIT Working Group discussed the origins and intended purpose of the Working Group; potential Working Group goals, objectives, and activities; and key barriers to Working Group progress along with strategies to overcome these barriers.
